Contact the PF office, the procedure should be online.. if not they might request you for a written application to request for details.. on company letterhead and authorised signatory signature
But first call the PF office and ask for procedure..

The Process is quite simple. Click on the Link Below
Employer e-Sewa
After the page is Open Click on Register to go to the Next Page.
Once you enter your Establishment Code and click on get details, you will get the following notification:-
{Your Establishment is already registered. Please check Establishment ID entered, and if correct download the Request letter format from the link and submit to the concerned EPFO Office under the signature of employer/authorised signatory. Click here to download the Request letter format.}
Click on the link to download the Application Format and submit the format after completing it to the EDP Department of EPFO.
