We are into Service Industry, This is the first time I will be conducting performance Measurement/Performance review for my organization,
the department covered are 1) Sales 2) Operations 3) Accounts 4) Data Entry Operator, 5) IT Department 6) customer Service Representative & last but not leat 7) Hr Department.
I would like to know what are the questionarrie to be used while conducting performance measurement/performance review for each departments.
In order to know where my organisation as well as as ours employees are heading

Hi Parab,
Firstly you need to know the goals of each deparment/person and accordingly prepare a review sheet. Write all the key goals against which they need to mark whether they have met expectations, exceeded expectations or not met expectations.
This needs to be filled in by each member and the respective department heads. Also ask them to elaborate on each points with reasons.
