From India, Madras
Dear Srinivas,

Form no.I is the statement containing the names of the ( not less than five ) festivals proposed to be approved as festival holidays for the industrial establishment submitted to the inspector for approval together with a copy of form no.II. Thus it is a one time submission at the time of seeking inspector's approval ( form III ).

Form V is the statement containing the dates of national and festival holidays falling in every calendar year with reference to the form III. It is thus a periodical submission to be done before the 31st December of the previous year. The festivals mentioned in the form should be the same as already approved in the form III by the inspector.

Form III and copy of form V shall be kept conspicuously exhibited in the industrial establishment.
