Form 10C is required for fully withdrawl of EPS accumulations under EPF/EPS & misc. provision act., Formula for calculation of gratuity will be Last drawn wages(basic+da)x year of service x 15 /26 for claiming grauity 5 years of service is necessary or 4 years and 240 days in 5th year of service is compulsory. For EPF nomination Form-2 is required and for withdrawl From 19 is required.
