I am in required of Evaluation form to get the feedback from my team members before leaving my current organization!
I have small team size of 25 team members and planning to quit.
It would be helpful some one would have shared the Team leader evaluation form or share the questionnaire about the same!
It is very much important for me to understand the area of improvement which is required for myself!

I have attached a form and hope you would find it useful.
You can modify it as per your wish.
Since you have not given your profession that has
not been covered in the questionnaire.
General behavioural aspects have been included.The form can be modified
and used with two rows. One self assessment and the other by his superior /peer.
There are plenty of 360 degree appraisal forms available and you
can get it by browsing in Cite HR Archives or from Internet.

I have tried in online and citehr as well however i have not seen any form as per my expectation!
