Dear Apoorva,
You have to fill form D once in a year or when ever there is a change in Basic or DA.
Mention the category of worker and there common slab salary. When there is a change in the wage then you can update the same in the register.
The importance of Form D is to show there is no discrimination between male and female workers in all cadres.

From India, Bangalore
Dear Sir,
I am still confused that as per Form D columns how we will fill the form
1. Is it fill by one by one every employee (Male & Female)
2. If not then what we fill in Category of works while we have appointed female workers in different categories like Security, Production Engineering, Production, Design etc.
3. Whom allowance will fall in Rate of Remuneration paid.
4. What amount we will show in Basic wages. Is it average Salary of all employees (& male, female will be saparately or not)
Pls suggest me.
Thanks
