Offer letter means company only select you for the particular job but not cofirm that your job is confirm i.e. company will appoint you or reject you for the job.
Appointment letter means company appoint you the particular job and your joining is confirm for the particular job.

hope your doubt is clarified by members in the precious comments. I wish to add in a layman's perspective.
Offer is nothing but a decision made as an out come of a discussion / meeting any one has with an Employer where he is satisfied and formally communicates that the position is offered under certain conditions. Here the recipient of the offer can accept / reject. similarly based on conditions, the person who makes offer can also reject once not accepted with in date. There is no relationship between the two when the offer is made.
Once the is offer is accepted and the person joins the Organisation, He gives a letter of appointment more relevant to nature of position he/she is appointed, KRA/KPI and other employment conditions. Here you have no option of accept / reject, because you have accepted the offer based on the conditions. By now there is a relationship is made as Employer and Employee.

As explained by Mr. Prabhakar, offer letter is only an offer to appoint you, it will be confirmed only after your acceptance, whereas the Appointment letter is issued to an individual after acceptance of the offer given to him. Offer will be given before assuming the duties to confirm your acceptance. Appointment will be given after assuming the duties as an employer.
