good effort, lot of thought behind it, just one question,, is it a draft or final one.. it look complicated a bit (at a glance).
Just a thought.. What are the policies regarding the resignation of the organization.. 'X' time in advance or immediate etc could be mention if needed..and on a lighter note,, in the end you forget to mention the fair-well party ;).
