I would like to inform you that, EPFO may asking the copy of Form 5, which you already submitted to the EPFO. At present all the employers are submitting their returns through online only, if it is the case there is no necessity of filling Form 5.
I think EPFO may asking the Form 5 for before the online period.
In your post you have not mention, which Month & Year of Form 5 EPFO is asking.
Anyway here I also have attached the Form 5 for your reference.
