Was an interesting link, will be helpful in future.
Maybe I should tell you the background of my request. My Organisation has implemented PMS in 1998, but it seems not as effective as the Org is expecting. Support from the employees are very poor. So, the Management would like to do a survey on what seems to be the problem. Is it the understanding, is it because they do not believe in the system, is it the tools provided is inadequate, etc. So, I am suppose to start collecting Questionnaire for the survey, and I found it difficult to coin out Questions and the answers....need help on this....

Then are you sure , in this case should you go for PMS survey or I think the aspects for which you want to survey comes under climate survey or more of job satisfaction.

Yep...I agree that it is more of PMS survey. The management wants to get to the bottom of the problem. Since 1989, so many programs have been introduced but lack support from the employees. Performance Appraisal Review (PAR), Performance Recognation In Development Enhancement (PRIDE) and many other that seems to work. So it is decided to understand the problem. Is it the tools and technique is difficult? or it is not communicated well, or people don't believe in it? or is it because it is not linked to wage? So, can you help me, please....
