Where are you studying and in which mode? Have you studied Research Methodology as a topic? If you have not, kindly see my thread on that topic. I have attached a ppt presentation there.
Have you conducted any literature survey to see what others have done, how and why that way? If not, I suggest that you do that first. That will lead you to some questionnaire items, which you can adapt.
