Dear Murali,
Please refer to the relevant statutes applicable to your establishment.
In West Bengal, in factories we have display Abstract of Factories Act relating to Hours of Work, Weekly off, Overtime and Payment of Wages Rules. We also have to display in Notice the Shift Timing, proper notices and signs relating to Health Safety and Welfare also needs to be put up.
This varies from establishment types and States.
Please give some more details.

Dear Aby,
There is no separate formats.The formats are given in the statutues itself. The abstracts are available in any shop which sells Forms, Formats and Legal Books.
As per law you have to maintain data in a format similar or exact to the format given in the Act.
