Hi,
You are doing a project it is good,but you want some one to help you out wite the questionnaire i would not apprieciate that.You can have a sample questionnaire and work on it.
Because i want you to think for you project and not to depend on someone else.
If you want we all can guide you with the way the questionnaire should be prepared but again you have to first research about your GARMENT organization and the organization work policies and the department they hold.
You first get to know about your organization and then think about the questionnaire.I have different type of questionnaire but don't want to give you as you will become dependent for all the things in future.
Think about it and if then also you don't get any solutions then i will surely help you out.
