Why not you can try and prepare a draft? It is for your betterment I suggest!

For Circulars there is no pre defined formats. Just you need to inform employees that use of of tobacco and mobile phone is banned within office premises. You should also explain why it is banned and effective date of implementation.

You also need to mention up front the penalties for non-compliance. No point having rules if there is no consequences for disobeying them.

Instead of putting circular on each and every issue which may have arisen in the workplace, frame some Standard Operating Procedures and exhibit in the notice board of the company. Issue warning memos to those who violate such SOPs. Do you have Certified standing orders in place? If not, refer Model standing orders and do the needful.
