Performance is based on Objectives. Have you got the list of objectives. Please remember, without focus on objectives, you cannot measure an employee's performance. There should be focus on revenue, earning should be based on objectives, objectives to be derived from Job Description where the Key Result Areas are indicated and these KRAs need to be analysed for Key Performance Indicators. Are you aware of all these ? Without these the Appraisal format will be just a piece of paper. Best wishes
