I worked for 10 years in an org and left that place around 30 months ago. I didn't have a great relationship with my supervisor (he was my manager for just the last 1 month in the org). Can I give my previous manager's name as the manager and reference?

In the background stated, it does not appear to be worth being considered as it is not a necessity that one has to mention the last boss for reference. You can list anyone whom you are confident of giving a favourable review.
