I am working as an HR generalist for an healthcare company. We have lot of women employees working in night shift and some of them opt to go by their own transport instead of company transport. I am required to draft a self-declaration to be signed by all women employees who opt for going self in night shift. Request you all to help me with a format of self-declaration for the same covering all the issues.
