EPFO UAN - Two employee UAN number interchanged due to miscommunication

Two employee UAN number interchanged due to miscommunication of UAN generation.all the persons are left the organization.
What type of form do we provide to EPFO office and how to sort out this issue.

Hi, Inform EPF officials immediately on company letterhead. However, most of such cases need to be settled mutually. For withdrawal form 19 & 10 C should be filled. Regards, Vishakha M.
