Please tell me the procedure of claiming Maternity benefits from ESIC.
1. What documents to be submitted to claim Maternity benefits and whom to be submitted???
2. What is the rate of Payment/How much they provide?? Is it be equal to my in hand salary or less than??
3. When i can claim?? is it before 6 six weeks of Expected delivery??
4. Is there any certificate of ceasing required from Employer??

Question wise answers are as under:
1. What documents to be submitted to claim Maternity benefits and whom to be submitted???
The IW is required to submit following documents to Local Office / Branch Office -ESIC:
(i) Not earlier than 15 days before the expected date of confinement, a certificate of expected confinement in Form 18 alongwith a claim for maternity benefit in Form 19 stating therein the date on which the IW ceased or will cease to work for remuneration; and
(iii) within 30 days of the date on which her confinement takes place, a claim for maternity benefit in Form 19 alongwith a certificate of confinement in Form 18 and a self-declaration pertaining to the details of her surviving children, as on date of presenting the maternity benefit claim.
2. What is the rate of Payment/How much they provide?? Is it be equal to my in hand salary or less than??
Rate of maternity benefit is roughly equal to the average daily wage.
Maternity benefit is payable for 26 weeks to the IW up to two surviving children and 12 weeks
for subsequent surviving child, subject to payment of contribution for not less than 70 days in the
immediately preceding one or two contribution period.
By way of the amendment in the ESIC Rules, an insured woman will be entitled to receive maternity benefit of 26 weeks of which not more than 8 weeks shall precede the expected date of confinement. However, an insured woman with two or more surviving children will be entitled to receive maternity benefit during 12 weeks of which not more than 6 weeks shall precede the expected date of confinement.
3. When i can claim?? is it before 6 six weeks of Expected delivery??
As stated above.
4. Is there any certificate of ceasing required from Employer??
There is no other certificate required other than mentioned above.
Note: the form number and submission date may vary from state to state. You need to confirm the same from your employer or local / branch office of ESIC.
