In my organization one employee died in November month, that time when i filled first page, it automatically submitted. but had not filled all accident summary in it. after that when i tried to modify option was unavailable.
1. how to i submit this report?
2. what should i write on letter head to ESIC?

Dear sir,
1st you just login to ESIC then check IP status weather alive or Dead. But make sure that you have entered date of death filed. Does'nt it a matter weather you successfully submitted accident report or not. To check IP status , just click on "UPDATE IP DETAILS" then enter deceased IP number after login into the ESIC. if you get status called IP dead then take a screen shot of it for future reference.
