Dear Swati,
You can share the employees a slide of PPT showing the purpose for these healthy habits.
Even you can engage a small awareness through a Doctor (if you engage a Panel Doctor for your company) on health issues.
You can display the pictures of healthy habits in notice boards.
If you are having an intranet you can share the same in the portal etc.
Even you can include the same in the orientation program when new employee come for joining.

Dear Swati,
It depends on what kind of industry you work. Please conduct an annual executive medical check and see the results. Once you have the result, you can plan what kind of awareness you need to create to your employees. Identify top 10 concerns from the medical report and socialize this to your employees. You can invite a doctor to speak about the health condition of the employees post the medical check-up and plan a yearly calendar on what you want to do. Similarly, ask feedback and suggestions from the employees. Your efforts will be successful when you do next year medical check-up and there are signs of improvements
