if the treatment is is done as per the panel doctor / dispensary of ESIC, then they will provide requisite form and send the member to AMO with the form and filling the form.
there is no alternate way to claim the expenditure done on dependent,
direct reimbursement for "accident only" cases is allowed for the the employee only, provided the establishment has submitted accident report within the prescribed time and the family members of employee has reported to the panel doctor / dispensary immediately after the incident.

If you are in Chennai please visit the Regional Director ESIC Office with copies of Bills Treatment Doctor fees room rent diagnostic expenses and medicine expenses.
Go with all details like Pechan Card photos taken by ESIC for identifying the patient, Form 6submitted on line which contains your number name number of days worked contribution of yourself and that of Company
Please go there on 2nd Wednesday when they organise Adaalat meet.
Go with a letter justifying the cause for external treatment when ESI Dispensary or Hospital is available. Genuine cases settled quickly. But medicine purchased as outpatient will not be Reimbursed under this scheme as it will lead to misuse. Hope you are clear
