Dear Sir,

My father is retiring from Gramin Bank. His PF is pending because in the nominee section, he listed his son, whose father's name title has been incorrectly recorded since childhood. When we inquired at the PF office, they mentioned that a joint declaration form from both the employer and employee is required, acknowledging the relationship as father and child.

I kindly request your assistance in obtaining the joint declaration form.

Thank you.

There must be any valid documentary proof of a child in which the father's name has been specified as per EPF records, such as the school final certificate of the child or the birth certificate. That will work with the joint declaration specified above. In the specified document, the name of the father (as per EPF records) and child must appear correctly.

change the nominee of existing employee is simple, fill form 2 (revised) fresh with new nominee and submit the same to epfo through employer.
