Thank you for sharing the information.
In fact this form was uploaded on EPFO website in the first week of November 2014 but with no instruction to it. Also there was no order or circular up till now. EPFO officials were up till now unaware of it. There was no clarification as to this new form additional one or replacing old form / forms. Now there is a clarity in this regards.
