Can some one give me a format of email to send to the employee who is taking excessive breaks a day at work?

Dear Thomas,
Before sending him mail, have you given him one-on-one feedback in private? By the way, do you have records of his excessive breaks? Where that employees spends the time in extended break time? How many instances of extended breaks are there? E-mail should be follow a personal feedback. Direct mails may leave that employee demotivated.
Is this isolated case or there are other also who take extended breaks?
Are you from HR or you are the manager of that employee? Has manager given the feedback? Have you trained your managers on "How to give Feedback"?
You have asked for format of the mail and in return I have asked so many questions. However, unless we know the background of the case, it may not be possible to give you accurate solution.
