Dear Shalini, Please click on the links in the sidebar at the right and you will find the discussions useful and also find some questionnaires. What course are you pursuing and in which mode?

Hi Shalini.
I can understand that internship is a very short period to learn new things but in order to gain most out of it, i'd suggest you to first form a draft and then seniors can help you out with their valuable suggestions and feedback.
Again as is stated earlier, we know nothing about your company and its people.
You know them as you're with them now for some time. Take help from your project guide at your company to know more about people and make a motivation questionnaire.
There is no need to differentiate questionnaires as per heirarchy unless your project guide wants you to.
Also while framing questions you should take into consideration -
1. Work hours and work-life balance
2. Male/Female ratio
3. ensure you have a few open ended question to know what motivates whom
4. since you're in your mba course, ensure that your questionnaire is not very basic and is fetching important learning points for you.
Hope it helped :)

Dear Shalini,
You have to frame diferent questionnaires for different category of employees (as per the company hierarchy) but remember to group them judiciously and keep them limited in number. Please note that the questionnaire is not very bulky and it ensures privacy to the respondent. On the questionnaire, on the top you have to mention that names need not be mentioned and invite for free and frank opinions.
