Hi,
The bank statement of HDFC Bank is a valid proof to attach. You can get the duly stamped statement from your branch. Even e-statement nowadays are a valid attachments.
With respect to your other query, I am not sure but have an idea that PF can be withdrawn after 6 months of completion from contribution date. But still, I would suggest you to verify with some seniors.

ok Just attach the "cancelled" check of the bank A/c. If epf deduction is less then 6 months then the EPS (part of epf) will be forfited. Stay Cool, Atom

Dear,
As said by Ashish, those are documents required for PF/Pension withdraw.
For your 1question as he dont have passbook, canceled check or other valid bank document which have bank address and account details will work.
For 2 question, no one is eligible to claim Pension if he have worked less than 6 months(as for pension calculations months are rounding off to zero for less than 6months and one year for more than 6months). But in case of Full disablement/ death there are chances that he/nominee can avail pension. Any one correct me if i am wrong! (I think he can claim PF,form19 but not Pension, form 10c)
