I do not think anyone will have a questionnaire and will give it you. You need to clearly state your aim, objectives, and any hypotheses you are trying to prove. QCs used to be popular in the 1980s and people have started to take interest again. I suggest that you kindly search for articles, digest the contents and prepare a questionnaire.
