Ask him what are the contributions for that particular period,value additions he has done,Response time,innovativeness in way of doing things,Where does he feel needs to improve,Team building? etc can be asked during PMS review.

Please see the attachment of performance appraisal which you help in your organisation.
Also you can discussed with all the HOD. Because every department has their different want of work. which you can add in appraisal form.
