Actually i want to know whether there are any Formalities to be done if any resignationm take place in any Organisation of the Employees, who is covered under the ESIC Scheme.
If there are any Forms to be submitted please let me know

As per my knowledge there are no forms as such for employees who have resigned and who fall under the ESI scheme. Whenever you are filing the half yearly returns you will automatically show the authorities that the employee has left the organisation on a particular date.
Half yearly returns are for the period 1st April to 30th september (first half ) and from 1st October till 31st March.
The forms will have to be submitted in quadruplicate to the local ESI office with a challan of the monthly returns which you will be havinng in your records. Kindly cross check with a senior experienced person as I do not know whether any amendments have taken place . Reason is I am currently not based in India. Hope your problem gets sorted out. As Mr. Sadashiv Rao said there is no form required to be submitted with ESIC authorities on reliving of a person who is covered under the ESI scheme.
In Half year return " Return of Contribution" Form No. 6 , there is a Col.No.7A which state that " Whether still continues working & drawing wages within the insurable wage ceiling" under this Col. you have to mention "NO" and also
Under Co.8 of Form No.6 under "Remarks " you are required to declare the "Date of Leaving" of the employee.
The information as declared by employer in Form 6 with respect to of leaving insurable employment by an employee will be forwarded by Local ESIC office to concerned ESI dispensary for necessary updates.

Then as u said amit if we fillup the particulars the employee will automatically receives the benefit of ESIC (money) or how?

Dear Member,
At the time of submission of Half-yearly return,(Form-6) of that contribution period, you have to mention against the name of the employee in a remarks column as (d-Deleted)
