Just to add my small contribution to this discussion.
The direct cause to an accident is normally an Unsafe Act or an Unsafe Condition that has been created which releases the hazard.Loss of CONTROL of a Hazard leads to accidents.
Using PPE WILL NOT prevent the accident but it WILL protect the individual or reduce the impact of the hazard release.

Just want to emphasise what I said with an example: Non use of PPE also can be a cause of accident... Looking forward to have a discussion in this if anyone dis-agree with me...
One person not wearing the helmet and working... Sudenly a bolt fell on his head... If he could have wear the helmet, he must not get injured... Hence non use of PPE can be a cause of accident... If he wears the helmet, this could have been converted into a Near Miss Case...
