Hi,
You do not need employers signature.
Just fill up the form and draft a letter explaining the same and address it to PF commissioneer at the local PF office.
All you will need is the employers PF Code and your PF No.

GET signature& seal of Astt.Commissioner of Labour or Govt Labour Officer or Gazetted officer (in place of employer) and submit the same to PF Office. It is PF Office's baby how to get informations from your Employer.Don't forget to fill establsihment Code and your PF Account no. in those forms.

If the employer refuses to sign on the PF withdrawal form, you can forward those forms to the concerned RPFC after mentioning your PF Account number and duly signing at the required places. Gazetted officers signatures are only taken when PF is to be withdrawal from the closed establishment. Otherwise in your case just forward your claim form to the concerned RPFC and then the PF officials will send thier PF Enforcement Officers to such establishment and then they takes some penalty and sign from your employer and then you will get settled your PF claim.

In this case you can directly submeet your pf form to pf office along with letter that the employer is not co-operating. then the pf office will directly send and your pf will be deposited to your bank acc.
