I do not have a format but I would like suggest that - you anyways have the candidates CV and its better to ask less number of questions and what you need to know about the candidate.

Please find the attached format for Interview assessment form. I hope it will help you. You can modify the same as per your requirement.Give the same format to candidates when they come for interview & let them fill their personal details/ educational qualification/ strengths/ weakness/ references.In same form you can also write your comments regarding the candidate & if you find him suitable as per your requirement, you can recommend him/ her for second round of interview with concern department.
