2. Your absence from ____ (date) to ___ (date) i.e. for ___ days is unauthorised. During your absence we did not receive any communication from you to justify your overstay.

3. This unauthorised absence of __ days is misconduct under the provisions of Section ____, Paragraph ____, clause ____ of the approved and certified standing orders.

OR

3. This unauthorised absence of __ days is misconduct under the provisions of Paragraph ____, clause ____ of the appointment order issued to you on _____ (date).

4. Management has taken serious note of your absence and you need to give explanation in written for this misconduct within 48 hours of receipt of this letter by you.

5. If it is found that the reasons for your absence were not valid or untenable or if no written explanation is received then management will initiate disciplinary action against you.

Please fill in the blanks and take a printout on your company letterhead. Take signature of employee on the second copy. Make sure that the second copy is also signed by the authorised signatory and you do not use photocopy of the original one.
