HI,I need to design a clearance form for the company because on of the employees has resigned and needs to be cleared before he is paid his dues, kindly help me with the format that i can use .thanxs

Clearance form is pretty simple. Its main purpose is to declare by each department that the person has no belongings with the particular department. So the form can be created in such way that the HR dept decleares that Mr/Ms xyz has resigned with effect from this date and request each HODs to declare if s/he has any belongings in the department.
Finaly it should come to HR which has to give the final detials of his nubmer of days worked in the month(for salary payment) and id and other documents relating to HR has been returned by the employee. Finnaly the form can be sent for accounts department for the final settlement.
