I have query that, one of our female staff is married but due to non avilablity of gazzet copy about change in her name, i had not changed her name in ESIC card.
Now the situation is that she is pregnent.Without receipt of gazzet copy shall I change her name on ESIC card?
wheather she will be able to get Maternity paid leave facility from ESIC by same name or not?

Hi, As per my knowledge, you can also change her name with the help of copy of marriage certificate alongwith application from her and supporting letter of Employer. Regards.

The ESIC department can not refused to provide the benefit in any situation yet she married or unmarried pregnancy. because she is paying contribution for same and eligible for all medically benefit.
Company should submitted the Form 1 with new details of her family like husband name mantionedand also attached her affidevit about marriege details (Both ID proof also attached). if BM refused than contact special cell of Regional office ESIC about this matter.
If she is adults than the department must be labile to provide the all benefit regarding sickness and maternity.
