Have you checked into the SA-8000 standard for social accountability? It might provide the basis for your explorations.

Could you use the SA-8000 standard, as well as current concepts in corporate social responsibility, as the basis for the areas / questions you would want to include when you craft your questionnaire?
This is a topic that I have increasing interest in, but none of my current research or clients have presented the opportunity to build this yet.

am trained on SA 8000, ethical trading initiative n world wide responsible apparel production n hv worked closely with different cos engaged in manufacturing sector....
if u want to do a real survey, position is dismal in several sectors.. ask any labour inspector/ trade union member, thye will give you the true story.. however i hv seen worker interviews wherein workers are chucked out next day enmasse...
"what is the future of your kids", this question if asked by a ngo from a worker's family member would spill all the beans..
