Thanks for posting the questionnaire. Is this something that your company uses?
From an academic's point of view the questionnaire is too simplistic and would not meet the objectives of a project report, as the most of the questions other than pen-ended ones are dichotomous (i.e. require Yes or No responses) and cannot be subjected to statistical analysis.
I would welcome views opposite to mine, to help generate a debate, which could be of use to students.
