Dear Manoj,
All photo ID for a company should be made according to the Factories Act 1948 rule 110-A, Form no 36 it should cover the following :
(a) Name and address of the Factory:
(b) The full name and address of the worker:
(c) Date of birth of the worker:
(d) Date of joining the service in the factory:
(e) Recent passport size photograph of the worker:
Signature or left thumb impression of the worker.
Signature of Manager or Autorised Agent.
Date of Issue.
