I would like to introduce mysely as MohanaPriya N, working as HR in a private IT Concern. We in our organisation has planned to motivate employees by selecting one employee everymonth and awarding the employee as "Employee of the month".
I would like to know the checklist to conduct Internal survey among employees on performance, Interaction etc. and also i would like to know what are the matrix required from HR point of view in this survey.

What you are talking about is nothing but "Employee Perception Survey".

To conduct these kind of surveys, your organisation should be quite matured one.

Instead of getting into these higher level HR interventions, first ask yourself - "do I do everything that HR Management theory tells me?". Answer to this query you will get when you do HR Audit. Plug those cracks that you get in HR Audit Report.

Secondly, have you defined departmental objectives for every department? Check whether each department works as per their process manual. If yes then what is the process variation? What steps do you take to overcome this variation?

Employee surveys though very good, are too serious businesses too. There is a very famous proverb in Hindi as well as other Indian languages "Hath ke Kangan ko Aarsi Kyon". Your exercise of survey should not fall into this category.

Employee Survey reports always startling outcomes. Does your management has will power to take corrective action? Otherwise this entire exercise will become futile.

How many employees?

If we want to motivate an employee, we need to work with that employee not the employee's coworkers.
Employers that reward one employee at a time do not motivate all employees.

Ask the employees' supervisors.
"Interaction etc."
A 360 may be effective.

The strength is 11(Staffing Division), I am working for our organization's staffing division.
I do accept that, to motivate employees, have to work with the employees and to ask for the supervisor’s evaluation feedback, but along with these, in order to have opinion’s from co-worker’s also (on performance, Interaction level among employees, Sharing information with co-workers etc), decided to go with internal survey. Kindly help me.
