I would like to know from the forum if any one has framed a template or plan for improving an employees' performance. If yes, would you mind sharing the same.

Hello Manoj,
The basic steps are:
With the employee, identify the performance shortfall.
Agree with the employee what good performance looks like, in measurable terms.
Agree with the employee a performance improvement plan.
The plan should include: measurable targets, dates by which targets will be reached, employee development plan, resources required.
Provide ongoing employee coaching and regular progress reviews.
