Shifali it depends upon the cash you have given to the employee if you have paid him the salary for 28 days then you will calculate the Bonus on 28 day's salary but if you have considerd the remaining days as leave and paid him full salary for the month then you will calculate the Bonus on full month's salary.

Although there is no provision for payment of bonus on monthly basis, bonus has to paid annually.
Yes, if you are paying bonus on monthly basis it will attract ESI contribution. Any payment which is being paid to workers regular beyond two month will attract ESI Contribution as per second part of definition of wages under ESI Act.
