No need to say about your agreement with the company and they (ESIC) is not concerned about these things. Only thing that you have to do is to go to the ESI dispensary to which you are attached to and consult the medical officer there. He will guide you the rest. They will not ask about your service continuance but they are concerned only about the contribution that the company has remitted for you and that will be available in the online portal itself. Since you have paid contributions in the past, it is your entitlement and right.

I think you have never utilised the services of ESIC and that is why you are confused of the medical officer, dispensary etc. You can see the name of the dispensary from your ESI id card (it should be a printed paper with your photograph with your family affixed therein and signed across by the employer. If you have not received the same, ask the HR for the e-pahchan card of ESI.

If they refuse to give it also, then it should be construed that something fishy has happened that they have not paid the contributions. Then you can go to the ESIC and complain of it. Rest they will take care!
