Your description of how to fill form No 19 and form 10C of EPF is good. But being a person having a little experience associated with statutory returns and filing etc, I have a suggestion. There is no column/ row/ cell which may be left out in any form/ return. If you "leave" address column in form 19 who will fill it?
If you are to write only "Resigned" as reason for leaving won't it be printed in the form itself? The reason for leaving may be resignation, or it may be retrenchment of employment or it can be resigned after getting married (in case of woman employee) or it can be resigned and left/ leaving India for settlement abroad, or it can also be expired. There are different treatment for each reason, that is in case of retrenchment, leaving India or marriage (woman only), death a waiting period of two months is not required. Therefore it can not be just resigned but there may be some qualifying words attached to it.
Apart from signature for declaring that the employee/ member had not been employed in any establishment for two months and one signature to be signed as authentication of what all are written in the presence of attesting officer/ employer, only one signature is required across the revenue stamp. No need to sign again below the stamp. Also the second signature being a declaration of non employment is not required in case the fund is claimed by nominee after death of employee, or if the member (woman) is withdrawing the fund after leaving the establishment after getting married or a member is withdrawing it after leaving for settlement abroad.
In Form 10 C also address of establishment where you worked is to be filled.
Without filling the code No, how to identify the person?
Question number 8 relates to a major matter. That is, are you interested to avail Scheme Certificate in lieu of withdrawal benefit? Many persons opt for scheme certificate, then how can you say to write NO? In case an employee is withdrawing PF after a service of 10 years, naturally, the answer to the question No. 8 should be YES and even if you write NO, the PF Office will correct it as YES or send it back.
You can not leave col. 9 because there you have to show details of nominees. Similarly col. No 10 is also relevant. Col. 12 is to be filled in case he is already getting pension. But it can not be No only.
