In every organisation, PF deduction is made on Basic Salary + DA
Suppose your basic is 100.00
DA 500.00
Total 600.00
Pf Deduction(10%) 60.00
Employee's contribution and Employer's contribution will remain same
Percentage of deduction may vary from organisation to organisation

If fresh is trainee then he is totally exempted from the provisions of the EPF ACt
If he is on muster-rolls and drawing less than 6.500/- he is covered under EPF ACt & draws more than 6.500/- per month, its his choice to covered under Act.
But i had not heard that none said he intend to cover under EPF.... however, in my opinion, fresh consent is necessary to cover him under the provision of EPF Act if his salary is less than 6,500/- PM

Once a person joins the company if his basic & DA is below 6500/- then he has to be compulsorily covered irrespective of his wishes.
Further, If a person has joined a company and previously been member he has to be covered under PF membership and can limit to his PF Deduction to BASIC & DA 6500/-.
If a person joining company without prior membership of PF and If his Basic & DA is above 6500/-, he need not be covered under the PF and he can be exempted and can declaration from the employee in FORM - 11

PF is deducted @12% on Basic + DA drawn and same amount is also contributed by the employer also.
Employee's Share full 12% amount is credited to PF account.
Employer's share 12% is divided in 2 parts.
1. 8.33% (Being credited to Pension Fund)
2. 3.67% (Being credited to PF account)

If that employee's Basic + DA is > Rs. 6500 there is no need of deducting PF contribution from his salary.
Otherwise put him in trainee.
He will be happy and company will be more happy.

Sorry to interfere with ASHOK_MODI1978's comments/version.
For the purpose of exlcuding an employee from the provisions of any law more particulary social welfare legislations, employee designation & salary should not be reducded/enchanced. Which is an offence under labour laws.
Further mere conferring a designation to an employee does not confer the status the NATURE OF JOB he does matters.

what are all the documents to be collected from employee for PF and ESI Deduction? Can anyone please help me out this problem?

Thanks you so much for your valuable suggestions
The CTC of that candidate is 15000/- Per month
Total PF contribution is 780+780 and the basic salary is 7500/-
If we will consider his request and not deducted his PF amount , than how much net salary we will going to give him .
As when we deduct the PF for the same salary amount than net salary is 13440/- but in this case if we will not deduct the PF part that is his salary be 14220/-( when only employee part will be giving ) or 15000/- ( both the employer as well as employee part will be giving)
Please suggest .

Dear Mr. Kumaresank,
I agree with you.
But take an example,
one person is drawing gross salary Rs. 15000/- Per Month
For him Company is deducting ESI @1.75% = RS. 263 and paying ESI Contribution @ 4.75% Rs. = Rs. 713.00
Now company increase his Gross Salary by Rs. 50.00 Per Month and that person comes out of ESI.
Here Employee saves Rs. 263.00 (In Cash) and company saves Rs. 713/- per month.
Is this violation of any law as per your above quote?
Any person can not deny from PF deduction which is also an offence.
You should be practical.
Company don't want to loose that person and that person don't want PF to be deducted.
In my opinion, with the conscent of both a middle way can be choosen.
Law is for the people and by the people. people are not for the law.
