What we do in our Comapny is:
Present a bouquet and the card signed by all the employees of the Company (we do have 250 employees in our unit) hence we take slightly big card which does not go to B'day boy/girl for signature. Then we all (on the same floor) gather near to B'day boy/girl work place and present card and bouquet.

well it can be like decorating the desk of employee before he arrives his office, that will be first surprise then as soon as he open his mail box a well crafted mail and then as soon as he settles a gift and a bookey,dont forget with a smiling face of HR

Go for b'day cap (get from fancy center). Invite all while fixing the invitation in your notive board.
Take out the snaps for your record purpose. then the same you can write in your MIS Report.

on b'day seen employee want holiday or they didnt cum...reduce the abcentizm on b'day,invite the person from the family lik wife,son...n falictae him/her with flowers gifts as per possible to company from there hand on company behaf,it will remind n motivate empolyee to come at work palce..

To add to what others have suggested, you can also hand over a Bouquet to the employee. This can be handed over by the senior most person in the organization, present at that time.

We have started a good tradition in our company...
gather the entire department and ask each member to spell out Good qualities about the Person.
* Its tough to spell out good as now days we normally talk negative.
So it would be a nice experience for The birthday Guy/Girl
Or give him a slam book with Quotes form every HOD and ask the person to spell out the Thoughts before cutting cake.
This will help person to identify the competencies in him and gain confidence and trust in his team.

You can order a cake for the bdayboy or girl, give card signed by all the staff menbers or if a big organization then by your head.Before your office over have bday celebration send a mail to everyone to assemble to your cafetaria or place where everone can assemble. And organize some funny games for abt 15-20 mints and offer some snacks to everone.

You can declare a day in a month for birthday bash and celebrate the birthdays of all the employees whose birthdays are falling that month. An awesome cake, some snacks and soft drinks should be the food. You can also go for buying some small gifts for the employees who are celebrating their birthday. The best thing to do is to declare a particular employee every month who will be organizing and hosting the entire party.
Some interesting games and DJ should also be planned. But stick to a time limit and try to finish it in that particular time limit so that work is not effected.
Some other indirect motivational activities can also be a part of the party.Like awarding the performer of the month.
