Have you done any literature search on this topic? There is a very short and effective questionnaire to findout if there is a WLB at <link no longer exists - removed>
The questionnaire posted by Vaideshwari is good, though long and needs fine tuning. For example, personal questions should be left to the end and the script should be subjected to grammar check (sorry Vaideshwari for pointing it out in a public forum). Also, you need to strike a balance between length and its utility.
