Lot of resources available online. You may refer the same and prepare a form as per your Organizational requirement. Appraisal form parameters will vary from industry to industry and hence please don't look for readymade form.

If you are a qualified HR person, you should know that Performance Management System is unique and different for each organisation. Goals are dependent upon organisational revenue. Goals vary from employee to employee who contributes to the revenue of the organisation. Revenue can be either tangible or intangible. Performance Appraisal form captures the Key Result Areas identified for each job role and are measured by Key Performance Indicators. As mentioned by my learned professional friend above, there is no standard format. And this is not the first time I have mentioned this in this august forum!

It's not practical to have an 'omnibus' format for reporting appraisal as various parameters on areas to be highlighted while doing so. There are practices of 'self appraisal' which is to be continued by assessment remarks to be recorded by the supervisor, TL, HOD and so on. However certain data such as name of the employee, Emp.ID, Designation, name of dept,, DoB, DoJ., Salary drawn, period of assessment etc can be common boxes applicable to all which will appear top of the facing sheet. As posted by other learned colleagues there are ready-to-use forms available on the net can be downloaded and used with modifications, to start with, if you would face difficulties in designing your own formats.
