There IS NO standard document.

Every company and organisation is different and working in different industries.

You need to write your own employee handbook and HR Policy manual based on what YOUR ORGANISATION requires. You are operating now. Write down and codify all your existing rules, policies and requirements and you have your manuals specific to YOUR ORGANISATION.

What happens in the organisations all our other members work for is not applicable to YOUR ORGANISATION.
