Under EPFO FORM-11 (REVISED) is a self declaration by the employee based on which employer will decide the status of employee under EPFO - Excluded employee or to be covered under EPFO. If covered under EPFO , the status of EPS deduction. All FORM-11 must be kept properly by the employer and in case of any dispute to be referred as document.
